Telemedicine visits are now covered broadly by most insurance plans, at least through the end of 2024.

The requirement that only special computer software can be used is now waived and your first visit may now be via telemedicine.

This means that we can now serve new and current patients with a convenient Telemedicine Visit.

We have adopted Zoom as our favored platform given that it is available for free download and is available for a wide range of computers, tablets, and mobile devices. 

Here is how to make it happen:

  • Call my office at 301-774-4400 to schedule an appointment.
  • If you are a new patient (or have not seen us for more than three years) download and complete your patient registration form.
  • Return your patient registration form, insurance cards, and copy of your identification (license, passport, etc.) securely via FAX to 301-774-1034 or US mail to our street address. Email to office@drkozam.com is effective, but may put sensitive personal information at risk, so it is not recommended.
  • Telemedicine visits are similar to in person office visits, although the ability to perform physical examination is limited. We make reasonable efforts to protect your privacy during the visit, but a highly motivated cyberattack could potentially eavesdrop or even interfere with your visit. Just as with other visits, insurance benefits are dependent on your specific plan, thus copayments and deductibles may apply.
  • Out of pocket expenses may be paid by credit card over the phone and are expected at the time of your telemedicine visit.
  • Click Start my Zoom Telemedicine visit to begin your appointment.  You may also enter meeting ID 997 134 3592.  If your are asked for a passcode use 4QLuGJ (case sensitive).
